SAMPLE ONLY - Selection Committee Criteria Form

A Scholarship Selection Committee often use a form to judge an applicant's qualifications.  The form is based on the requirements the applicant must meet in order to be considered for the scholarship.  A point system is often used.  Points are totaled after each Selection Committee has reviewed and judged the Scholarship Packages.   The applicant with the highest score will be awarded the scholarship.
